A patient registration form is used to collect information and admit a patient into a hospital. The forms used for new patient/client registration are valuable because they contain all sorts of information.

However, many hospitals that ace in their healthcare and treatment lack a couple of major aspect in their management system, which is a good form management tool and an efficient patient registration form. These forms hold value as they act as basic guidelines about a patient’s overall medical and general background.

Therefore, it is vital to have an efficient online registration form with a precise layout.

How?

For example, a registration form for an eye clinic will have a different layout than a registration form dedicated to an orthopedic clinic.

Both these forms will have different fields regarding consultation, referring doctors, medical tests performed, etc. Not only this, but a patient registration can be of many types;

let’s look into some

Types of Patient Registration Forms

  • Regular Forms For Admission

The first type of form is the regular registration form. These forms are required to admit a patient into a hospital. This form can contain fields like name, address, gender, contact details, date of admission and DOB of the patient, payment information, facility availed, medical condition, and to conclude the form, an agreement to abide by the hospital policies, a.k.a T&C field.

  • Forms for Performing Medical Tests

Some forms need patient’s approval for conducting medical tests for them. These tests can be of various types, such as blood tests, fluid tests, and many more.

Apart from the general details, such forms ask for information like a list of allergies, medications prescribed, patient’s approval statement, and their signature, etc.

  • Consultation Form

Consultation forms are a bit different than the other forms. Some patients need an appointment only for a consultation. Creating an account just for one consultation adds no value to their experience.

Therefore, such forms do not require any additional registration; filling out a consultation form is all the user needs to do. These forms have fields such as name, address, preferred consultation time, preferred doctor, department for consultation, and so on.

  • Medical Authorisation Form

Some forms need approval and authorization of patients regarding their registration into an online patient system. Hence, these forms mostly have T&C fields that a patient needs to agree on.

  • Patient Background Form

While admitting a patient into a hospital, they fill a form that mentions every information regarding their medical and general background. These forms mostly contain checkboxes, dropdown menus, text fields, date fields, and contact details.

  • Patient Insurance Form

Insurance forms are a vital part of the admission procedure of a patient. While the medical forms cover a patient’s general and medical relevant details, insurance forms cover the payment.

These forms contain fields asking for the patient’s employment details, bank details, insurance providers and similar other related information.

  • Discharge Form

While discharge forms are not exactly patient registration forms, they are a vital part of the overall medical forms. Discharge forms contain discharge timing, medicine prescriptions, a list of treatments, and much more.

Which plugin to use to create a patient registration system?

Creating these forms needs an efficient plugin. Moreover, if you wish to create an ecosystem for your visitors, using only one plugin might not be sufficient.

To create such efficient forms, try using RegistrationMagic. This plugin allows the admin to create forms with different layouts. Be it a multipage form or a single page detailed form, RegistrationMagic takes care of everything.

Not only this, but the admin can also keep track of the form analytics and field analytics separately.

Create Patient Registration Form on RegistrationMagic for Free!

Another preferred plugin is ProfileGrid Memberships. This plugin allows your users to create user profiles on your website. Not only the users but profiles for all the hospital staff can be created using this plugin.

With the help of this plugin, the doctors, nurses, and hospital staff can create their user profiles and the patients can have their own memberships or subscriptions.

Type of Form Layouts

There are three ways to represent the patient registration form;

  1. A Multipage form layout
  2. Single page form with a detailed layout
  3. Multipage form with a detailed layout

Customization of all these forms is similar; the only difference is that you have an option to design each page of the form separately.

In this article, we have discussed the patient registration form sample on a single-page layout with a step-by-step guide on creating this form. Refer to this template and the guide to make a simple yet efficient form for patient registration.

Guide To Create a New Patient Registration Form

Step 1: Click on the ‘RegistrationMagic’ plugin

  • On your dashboard, search for the RegistrationMagic plugin.
  • A dropdown menu with many options appears.
  • Next, click on “All Forms” present at the top of the dropdown menu.

First step to create a patient registration form

  • A new “All Forms” window opens on the right-hand side of your screen.

Step 2: Adding Name to the Form

  • Under “All Forms,” a menu bar is present that contains various options.
  • Click on “New Form” present in the menu bar on the top left.

  • Next, give a name to your form.

  • Since we are creating a patient registration form template, we will name the form “New Patient Registration Form
  • Then click on “Save” so that your patient registration form is saved under this name.

Note: Don’t tick on the “Turn off user registration for this form” option as it will prevent the users from registering.

Step 3: Finding the “Fields” option

  • Now that the form is created hover over the cursor on your patient registration form.
  • Two buttons will appear as soon as the cursor is placed on it.
  • Click on the white button that says “Fields“, and you can see a new window with various options in the menu bar.

  • Now, click on “Add Field.”

Step 4: Steps to add various fields and add them to the form

From here, choose all the relevant fields that you need for your registration form.

We have highlighted a few important fields that we think would be important.

Field 1: E-mail

The “E-mail” field appears by default on a RegistrationMagic form. The user can enter a valid e-mail ID on this field. Using this data, the hospital can send updates to the patient about the registration process.

Field 2: Username

RegistrationMagic has an option with which the admin allows automatic account creation while the user registers with this form. Hence, the user is free to add a username to their account.

The “Username” is a default field in all RegistrationMagic forms. You have the option to either keep the field or remove it.

Field 3: First Name

The “First Name” field allows the user to enter their first name. You can further customize this field by clicking on “Icon Settings” and “Advanced Settings”.

Field 4: Last name

Likewise, the “Last Name” field has similar functions to the first name field; only users must enter their last name in this field.

Field 5: Date of Birth

The Date of Birth field is relevant to determine the age of a patient. A patient’s age has a key role in medical practices to finalize the modus operandi of a line of treatment. It is created using the “Birth Date” field.

Field 6: Gender

The gender field allows the user to fill in their gender. This field has a similar significance to the DOB field in a medical registration form.

This field is created using a “Radio Button” in which the user will select only one option at a time.

Field 7: Admission Date

The admission date is created using the “Date” field in which a calendar appears when the field is clicked.

The user can use the calendar to select the month, year, and day of admission to the hospital.

Field 8: Current Adress

The current address field is created using the “Address” field. This allows the user to fill out their current address.

There are two ways with which a user can fill in their address. One is the regular option in which the address has to be filled manually by the user.

The other option is to use the Google Map location pin to auto-generate the area where you live. RegistrationMagic has an integrated location access setting which is powered by Google maps. This makes the task a bit easier for all the users.

In manual settings, the address fields have checkboxes that say “Required.” If you check those boxes, the address fields become a compulsory field denoted with a red asterisk sign “*“.

Next is an eye sign with which you can adjust the visibility of the “Address Line” fields. A crossed-eye sign denotes that the field will not the visible or functional on the user end.

Field 9: Contact Number

Contact number is an essential field in every form. In this patient registration form template, the “Mobile Number” field is used.

Field 10: Pre-existing Ailments

This field is created using the “Text Area” option. With this field, a user can enter data in the form of paragraphs using all the characters.

If you wish, you can also place a hover text for this field to guide the user on what to fill in the blank area or how to fill the information.

Field 11: Recent Prescription

This field is created using the “File Upload” option. This field allows the user to upload files in the format set by the admin in the back end. The file can be anything ranging from an image to a PDF or word document.

All you have to do is specify the acceptable file formats and add that to your form.

Field 12: Type of Patient

Hospitals have categories for different patients. Hence, to fill this field, it is created using the “Drop Down” field. In this field, the user will see a drop-down menu with different options.

Field 13: Marital Status

This field is created using the “Radio Button” field that functions similarly to the gender field.

Field 14: Guardian’s Name

The guardian’s name is created using the “First Name” field with the field labeled as Guardian’s Name.

Field 15: Guardian’s Contact

Similar to that of a contact field, this is created using “Mobile Number“.

Field 16: T&C

Like every official form, this patient registration form should have a ‘terms and conditions’ section specified too. Hence, according to the hospital’s policies, the user must read the portion and click on the ‘I Agree” checkbox.

Field 17: E-Sign

E-sign is used to take digital signatures for authentication and validation of the form by the user.

The final step is to click on the “Submit” button, and the user will submit the form.

Step 5: Copy the shortcode

  • From the “All Forms” page copy the shortcode of the form that you just created.

For example, in our case, it is the “New Patient Registration Form”.

(You can copy the shortcode anywhere across your website. The from is, however, published and visible on the front end of your website.)

Moreover, it is on us to make your job easy. This is one of the many ways with which a registration form is published

  • On the left-hand side, click on “Pages” and then “Add New”.

  • This will open a new page window.
  • Add the title of your form on the title bar and paste the shortcode in the main body.

Step 6: Publish the form

These were some easy steps to create a free online registration form using RegistrationMagic. This form is the basic version of how a patient registration form looks like. All these steps cover the back-end work to create a registration form for the patients.

Here is how your form will look like to the user.

Creating any type of registration form is pretty easy. Getting relevant fields with an excellent plugin is all you need to create a form for your online patient registration system. Online forms serve you the ease of avoiding long queues to fill the form.

Hence, try out RegistrationMagic to easily create an efficient patient registration form template and publish it on your website without any hassle. Also, try ProfileGrid to create efficient user profiles for your website.

Add WordPress Form Number Field with RegistrationMagic
One can use the WordPress form number field for taking user data in numeric values such as unique identification number, account number, etc on the registration form. Number fields are extremely essential to create a complete registration form aimed at taking only authentic users.

A WordPress form Number Field accepts numerical value inputs only. Users cannot add alphanumeric texts to this field. So, this field in a form declines any other value confirming whether the user is entering a correct data.

To broaden the scope of your WordPress registration form you need to install a form building plugin that offers custom fields to add to your form. RegistrationMagic is one such plugin that offers an array of custom user fields including the WordPress form Number Field.

RegistrationMagic adds a variety of WordPress new user registration solutions to your site. Your site can buy and sell products, offer newsletter subscriptions, assign user roles etc. all via these forms. The custom fields of this plugin, help to create such dynamic forms. Users can also add space between WordPress form fields to design a neat content heavy form.

So, let’s proceed to add the WordPress form number field to online forms with RegistrationMagic.

Navigating to the Number Field

From your WordPress dashboard, go to the “All Forms” section in the RegistrationMagic Plugin menu.

WordPress form number field navigation

Hover over a form cover and you will find the “Fields” link along with Dashboard. By clicking on the Field link go to the “Field Manager” page.

WordPress form number field dashboard

So, now here, you click on the Add Field link at the top corner of the page to see the wide range of fields from RegistrationMagic.

A pop-up will appear consisting of all the predefined Fields of this WordPress profile plugin.

However, any new form that you build with RegistrationMagic comes with three mandatory fields email address, username, and password. Along with them, users can choose to customize their forms with these custom fields.

So now, you will be able to see all the fields sectioned under different subheadings. Click on the Number Field under Special Fields.

WordPress form number field number field

A page titled “New Field” will appear. Here you will find different fields namely label, placeholder text, hover text, icon, and advanced settings.

Tips: Even after choosing a certain field, users can change the Field Type from the New Field page as well. A drop-down beside the heading provides a list of all the available fields.

Configuring your WordPress Form Number Field

Here the Field Type stays as “Number” since we need this field to allow the users to the only input value in numbers. Name the Label as something which will help you recognize the number field on the frontend. In our example, the Label is “Unique Identification Number”.

WordPress form number field edit fields

The Placeholder Text field holds the text which appears inside the input box just before the user enters the value.

The Hover Text section is to provide a basic description of the field. In our example, we have filled it with “Accept Numerical Values”.

Navigate to Icon Settings. This will create an Icon just before the field where the user will enter the text.

Next options are Icon Colour and Icon Container. Icon Color provides a particular color to the icon. Icon Container chooses the background color of icon container.

Container Capacity and Container Shape determine the capacity and shape of the icon’s container.

Lastly, scroll down and click on Advanced Settings. Here, you will find a lot of fields incorporated. Let us take them up one by one.

WordPress form number field advance settings

Advanced Options

You can add CSS class attribute from here. However, the HTML must be a predefined attribute.

Then, assign the maximum number of field length as per your need. This will set the limit as to how many digits can be entered in the input box.

Next, the “Required Field” denotes that it is mandatory to fill this field.

Mark the checkbox and move on to the “Accept only Unique values”. This field determines whether you are inserting the same value again and again into the form.

Set the message as “This entry has already been used. Please use a different one”.

The next segment “Add this field to User Account” showcases this value in RegistrationMagic’s “User Manager Area”. It displays the value on the frontend of the user account developed by RegistrationMagic’s shortcode.

With RegistrationMagic, users can display WordPress form using shortcode. These shortcodes are embedded on the cover of every form and are unique to them. One just has to copy these shortcodes and paste them on a new WordPress page to publish a form on the frontend.

The last checkbox is for admins to decide whether they want to allow users to edit their data inputs even after they have submitted the form.

Finally, after configuring all the necessary settings on this page, click on the “Add to Form” button.

Configuring the Number field into your WordPress form

Copy the shortcode from the form card and navigate to Pages on your WordPress dashboard.

WordPress form number field copy shortcode

Click on Add New and paste the shortcode in the body section of the new page created. Click on Publish and your form is now live on the frontend.

So, your Number field set up is complete now. Now, go to the frontend of your site to see your form with the Number Field applied to it. The screenshot shared below shows how your Number field will look on your registration form.

WordPress form number field frontend

Furthermore, admins can create a custom user area page and make all the user inputs in their registration forms appear on the frontend. Here they can access their user account and edit them whenever they wish.

Tips: The field section in RegistrationMagic offers the option to add products and total price on a WordPress form. Therefore, you can display WordPress form with payment option and thus carry out the online transaction via Stripe etc. through it.

A Plugin for Diverse Needs

So you see, RegistrationMagic is your answer to add more value to default WordPress forms. Thus helping site builders procure diverse data from their users. Moreover, they don’t even need to create a full-fledged site to register users. This WordPress registration form plugin helps users to create intelligent contact forms for site owners to gather contact details of their visitors.

Further, this plugin makes it really easy to register members for courses, classes or workshops. Where they can also choose to restrict user access by date in WordPress forms. Added to that, there is also an entire page dedicated to designing forms in an interesting and fun way.

So build your custom registration forms and attract more users to your site. Thus create more opportunities for your online business and earn more revenue with your user base.

Add Input Mask Fields with RegistrationMagic
You add Input Mask fields on WordPress forms to hide user data that are not meant for the public. Since your users trust you with their data it is your duty to maintain their privacy. This way you can gain user confidence so that they keep coming back to you on the ground of reliance.

Users often enter confidential data while filling up a registration form. As a site admin, it is your job to secure that data and not expose it to others. To do so, you need to customize your WordPress forms so that they can take extensive user data. You will find several WordPress form builder plugins to add extra user data fields on your form. But only a few will allow you to use the Input Mask field to keep user data private.

The RegistrationMagic plugin offers Masked Fields to add to your WordPress forms. Masking is mainly done to protect data, which might be sensitive or personal. User data ranging from phone numbers, dates, card details, account numbers or tax ID can fall in this category. As an extra security measure, you can mask confidential user data so it stays invisible in the browser.

As I told you earlier, RegistrationMagic lets you add custom Fields and Widgets to make dynamic forms. The Fields will help you to gather user data while the Widgets let you display data fields on the form.

The Input Mask is also a custom field of this WordPress form builder plugin. So let’s learn how to mask user input in WordPress forms with RegistrationMagic.

Navigating to the Masked Field

First, install the RegistrationMagic plugin and activate it. You will then find the plugin menu on your dashboard. From here, click on the All Forms link to land on the All Forms page.

The All Forms page shows you all the forms that you create through this plugin. Now, as you hover your cursor over each form cover, you will find two links at the two bottom corners. These are Fields and Dashboard.

When you can add metadata to WordPress form you can show various information about the form to your users. This creates a transparency about your site thus making it more reliable to your users.

Visible metadata on WordPress forms makes your registration page look more efficient. But adding a user meta is not possible in default Google forms or free online form templates. So installing a WordPress user registration plugin would be a good idea.

RegistrationMagic is one such plugin that can help you create custom forms to serve diverse purposes. This plugin offers numerous custom fields to add to WordPress forms. WordPress metadata is one such field among its wide range of preset fields.

The user meta field also keeps track of all the activities carried out in relation to the form. Besides, when you add metadata to WordPress form, it helps to analyze the form in a better way.

So, let’s go ahead and learn how to add metadata to WordPress form with the RegistrationMagic plugin.

From this pop up, as you can see, select the Form Meta Data field under the Display Fields heading. After doing so, you will land on the Form Data settings page.

Form Data Settings Page Features

The Form Data page contains a lot of interesting modules that you can add to your form.

Add Metadata to WordPress Form form data settings

Firstly, name the label as User Data. The label for most of the display fields do not appear on the form.

Below the Label field, there are other settings with checkboxes. The “Number of form views” option displays the number of times the form has been viewed. Upon checking it, a text field will appear where you can insert a text related to the form view count.

In our example, we wrote the “Number of Form Views” to the ‘Text Before’ field. You can also add new text, which will appear after the form view count.

The next field displays the number of times the form has been submitted. Checking it, a text field will appear where you can insert a text related to the form submission count. In our example, we added “The total number of submissions so far” to the ‘Text Before’ field. You can also add new text, which will appear after the form submission count.

Scroll Down and you will find a checkbox with the option to “Display form submission limits”. Make sure you have set them on Form Dashboard. The next field, “Display Form Date Limits” will record a timeline of form submissions. This data also has to be set accordingly in the Form Dashboard.

The next option is “The last submission received time”. This indicates the time at which the form was last submitted. You can even include new text, which can appear both before and after the last submission time data.

You can also include the form name and a brief description of your form.

Lastly, you can apply a CSS Class that is predefined in the CSS coding file.

Frontend View

Now the only thing left is to publish the form on the frontend to see if the WordPress user meta field is working on our form. Since RegistrationMagic makes it really simple to publish forms with shortcodes, we will copy this form shortcode from the from cover.

Add Metadata to WordPress Form forms dashboard

Then we will go to the All Pages section of the WordPress dashboard and paste the shortcode on a new page. Name it whatever you want and click on Publish.

Add Metadata to WordPress Form publish form

Then go to site frontend to see if you could add WordPress metadata to WordPress form.

Add Metadata to WordPress Form frontend

You can see on the above form, there is WordPress forms meta at the top of it. It shows the number of views, submissions etc. the way we configured at the backend. This number will update on itself.

In a similar manner, users can also add space between WordPress form fields to make content heavy forms appear neat. Spacing is another field in RegistrationMagic like user meta and one can apply it anywhere in WordPress forms.

To get insight about the multiple login options of RegistrationMagic, refer to the WordPress user login plugin guide.

All About WordPress User Meta Tags

The meta description tag is a part of the meta elements of a web page. The meta description stays hidden from your visitors when they view your site but is accessible by the bots. One can display the contents of their meta tag description in the search results of the search engines or in a site description in a directory or data site where they scan your meta tag description.

Some content management systems and eCommerce software may also use this data when displaying a summary page like an articles list or a list of categories.

One Plugin, Multiple Utility

RegistrationMagic not only lets users add metadata to WordPress form. Its large spread of custom fields lets users create extensive forms with even payment options and product addition. So, users have numerous options to transform their sites into efficient online stores or membership communities.

You will see options such as Label, Latitude, Longitude etc. First, fill the Label field with the text that you want to appear as the field name on the form. I have filled the field name as “Office address”.

Next, set the Latitude and Longitude of the location that you want to show on your form.

There are two methods to apply your address here. One method is to enter the address manually. This action will present you with address suggestions.

As an example, if you type 1A then all addresses starting with 1A will appear as suggestions. The second method is to directly click on the map and select the exact location.

Both methods will auto apply the Latitude and Longitudes in their respective places. Saving you the trouble of writing them manually.

Then, set the Zoom and Map width as per your design requirement. You will find the unit of applying these features, written on the right side of the page.

Next, apply the CSS class only if it is defined in the coded file. Finally, click on Add to Form to finish up.
